SHOALS EARTH DAY VENDOR REGISTRATION FORM

1. All vendors should be self contained and are required to obtain all State Health Department permits, licenses, liability insurance, etc.

2. All Health Department certificates, licenses, etc. must be displayed at each vendor site.

3. No alcoholic beverages may be used or sold on park property.

$25.00 fee MUST be sent to :
